Hospital is very old. Equipment fails regularly. There is no security for staff after hours or to help with aggressive patients. Wards are chronically under staffed.

Rigid heirarchies between factions. Medical, Clinical, Support Services. Very resistant to change and personal accountability for performance. Leadership lacks clear vision and unable to define clear medium and short term goals. Stifled by a compliance focus over a capabilty and outcome focus. Supports inappropriate communication methods and poor behaviours. Government funding and planning processes drive wrong leadership/operational behaviours.
