Care Facilitator

About The Role

BaptistCare is seeking a full time Care Facilitator to join our Home Services Mid State team. Our team delivers high quality care to support our clients’ independence to remain living in their own home. You will provide coordination of services and support to the manager including supervision and leadership to care staff. The successful applicant will have a home based office and will be supplied with a car, laptop computer and mobile phone.

What We Are Looking For

The successful applicant will have relevant qualifications and/or experience in a disability or an aged care or home care related program. You will have a strong customer focus and demonstrated experience or ability in leadership, case management, quality management and team work. A current driver’s license is essential.
