Home Care Packages Consultant

Full Time/ Maximum Term until 30 June 2019!

You will:

  • Be part of an organisation that is bringing together Home Care Packages from several agencies as part of the Uniting merger.
  • Support the consumer to have responsibility and choice for the funding.
  • Ensure client care plans are consistent with person centred practice.
  • Coordinate and implement effective client care plans.
  • Organise referrals and liaise with service providers of support services.
  • Monitor, review and document care needs with clients and carers on a regular basis.
  • Enter daily progress notes on the Client Information System (CIS).
  • Meet funding and program targets.
  • Raise the profile of Uniting with presentations and attending forums.

Your experience:

  • Case management of services to older people or people with a disability..
  • Implementation of service delivery models.
  • Assist clients to increase their confidence; provide choices and develop flexible, capacity building plans.
  • Ability to source innovative, cost effective and creative solutions to meet client needs.
  • Ability to monitor and implement budget according to program guidelines.
  • Ability to use crisis intervention frameworks and strategies.

Understanding of My Aged Care and Home Care Packages is required 

About you:

  • Qualification in Social Work/ Welfare/ Health or equivalent..
  • Understanding of My Aged Care and Home Care Packages is required,
  • Ability to speak a second language i.e. Italian, Greek, Mandarin or Cantonese can be advantageous with client demographic..
  • Ability to source innovative, cost effective and creative solutions to meet client needs.
  • Excellent interpersonal and communication skills.
  • Excellent understanding of current trends and practices in supporting elderly people/ with a disability.
  • Excellent knowledge of relevant legislation and its impact on the Sector.
  • Ability to build respectful relationships with clients of diverse backgrounds
  • Motivated, confident and friendly.
  • Team Player and can work autonomously.
