Program Officer - Older People

Brisbane South PHN has an exciting opportunity to help shape the future of Older People’s health in Brisbane South. You will be part of a collaborative approach to strategic planning in the region to better support older people to access primary care and community services.

Assist in the planning, development, implementation and monitoring of the Older People’s Health and Wellness Programs, to improve health outcomes for the Brisbane South community.

Ensure activities focus on the achievement of program goals and contribute to the realisation of the Brisbane South PHN strategic goals and vision.  This is a full time, 12 month temporary role.

Brisbane South PHN is a dynamic primary health organisation concerned with improving whole-of-life health and wellbeing across our region through a well-coordinated and collaborative primary health sector, particularly for people at risk of poor outcomes – our vision is Better System, Better Health.  Brisbane South PHN is focused on partnering with commissioned service providers to facilitate an integrated approach to the delivery of timely, flexible health and community services in local communities for local people through our key projects.  Our work with Older People is growing and involves collaborating with a range of regional community organisations to facilitate strong, productive and outcome focused relationships to ensure our programs deliver optimal outcomes for the community.

To achieve success in this role, you will need:

  • Experience with Older Persons health care.
  • Engaging written and verbal communication skills.
  • High level capacity to work with relationship management and program computer systems.
  • Moderate level of Microsoft Office literacy.
  • Demonstrated ability to build and maintain positive relationships with internal and external stakeholders.
  • Capacity to work within a fast paced environment and respond positively to change and development.
  • Capacity to comply at all times with organisational policies and procedures.
  • Strong customer service skills used in an environment where high levels of empathy are required.
  • Capacity to work autonomously and independently within a values based team environment.
  • Eligibility for a positive National Police Check.
  • A good understanding of and experience within Health Sector particularly service delivery for Older Persons
  • 2+ years' experience in a similar role desirable.
