Bridges Health and Community Care is a leading provider of Mental Health, Drug & Alcohol & Offender Services across Bundaberg, Fraser Coast and North Burnett, operating in the region since 1997. We now provide services up north to Gladstone, west to Cherbourg and Kingaroy and south to Maroochydore and Gympie.  Bridges offers a variety of interventions and supports for all age groups including psychological services, outreach, counselling, education and training, healthy lifestyle and life skills programs and psychosocial rehabilitation. Bridges are committed to assisting the most disadvantaged and marginalised members of community, including indigenous, children and those with complex needs.

The Role:

We are seeking an experienced professional to work with offenders to assist them to successfully re-enter the community post-release from Qld Correctional Facilities.

Community RE-entry Services Team (CREST) workers work in Correctional facilities to assist offenders to develop  comprehensive individualised re-entry plans, provide information and referrals to relevant services to meet their needs and work in partnership with approved providers to ensure success on release.  CREST workers also work with offenders on release in partnership with Probation and Parole offices in the region, providing post release and crisis support for eligible offenders and co-case management for complex needs clients and youth offenders. 

This role is to provide services post-release ONLY i.e in the Community and is based in Bundaberg. Services are provided at the Bundaberg Probation and Parole office or at Bridge's location/offices. The CREST worker will work closely with the Central Qld CREST provider, Lives Lived Well in the delivery of the service in accordance with our sub-contractual arrangement and agreed KPI's. 

A vehicle for work purposes is provided.

The Person:

Applicants for this position will have experience working with offenders, excellent communication, assessment and case management skills, accurate, timely data entry and case notes and the ability to work collaboratively with all key stakeholders.

Tertiary qualifications in human services or similar are preferred, alongside additional skills training relevant to the target group.

Key Selection Criteria:

KSC1: Relevant qualifications, skills training and experience in working with offenders.

KSC2: Demonstrated ability in assessment of needs, goal setting, case management and crisis interventions and a thorough understanding of the needs and issues for offenders, including socio-cultural factors, to successfully transition them back into the community.

KSC3: Demonstrated ability to engage successfully with all key stakeholders, including families.

KSC4: Excellent written and verbal communications skills, use of contemporary IT systems, timely accurate record keeping, case notes and data entry.

KSC5: Current driver's licence.

All applicants must undergo and successfully pass a comprehensive criminal history check conducted by Qld Corrective Services and complete custodial awareness training prior to working with offenders.

Remuneration commensurate to qualifications and experience.
