This position sit in the HIP program and is a collaborative strategy between Northern Health and DPV Health. This position seeks to provide timely and quality clinical and social assessment treatment and planning support to reduce the incidence of clients with complex care presenting at Northern Hospital Accident and Emergency, via an effective outreach community mental health support service

Applicants must have:

Applicants must hold tertiary qualification as a Social Worker (Grade 3) or a Psychologist (Grade 2) combined with a Post graduate qualification and/or experience in mental health. Your knowledge and demonstrated skills in the crisis intervention for people in social and psychiatric crisis along with your experience in a Community Health setting will be highly valuable for this position. A working with Children’s Check is required for this position.

Proven ability/high level of interpersonal skills to effectively liaise, consult, negotiate, communicate and collaborate with clients, their families and other agencies and service providers.
