Care Manager- Aged Care

  • 3 week role, full time hours
  • Outer East Location
  • Great Opportunity

 

Our client is a provider of careers to aged clients whom currently still live at home in the outer eastern suburbs of Melbourne. We are currently looking for a case manager to work on the delivery of home care packages to clients for a three week period. 

 

To be successful in this role you must have;

  • A qualification in aged, nursing, social work, allied health or related discipline.
  • Home care package experience
  • Experience in identifying, coordinating and managing care services for aged, community, health or social services
  • Knowledge and experience in managing government subsidised home care packages preferred
  • Demonstrated high-level administration skills
  • High level organisational and time management skills

 

In this role you will be required to;

  • Conduct care consultations with potential clients – private and home care packages – to understand their goals, care needs and personal preferences
  • Develop and manage clients care management plan
  • Perform regular client reviews to ensure quality care services continue to meet the client’s needs
