About the opportunity

This is an exciting new role which will provide OT service provision to clients of the Street to Home (S2H) Program, as part of the recently DHHS funded Bacchus Marsh Modular Unit Program. The position provides in depth assessment of clients living skills and social identity, who will be housed primarily through the modular unit program to determine independent living readiness while supporting clients to gain these skills

About you

Applicants for this role will have a thorough practical working knowledge and understanding of person-centered approaches to working with clients who have a history of sleeping rough. Clients will be housed in the Modular modular Unit Program. You will be highly organised and possess a comprehensive knowledge of supports and services available across health and community services. You approach your work with a strong commitment to working proactively and are passionate about working with people who may have significant social and mental health issues.
