About the opportunity

The Home Based Care – Case Manager will assist in the delivery of high quality, innovative  Home Based Care (HBC) placements throughout the Wimmera. The position will case manage young people on statutory orders who are placed in HBC – with a focus on foster care.  The position involves ensuring children are thriving in HBC and to support their carers and families.

About you

The successful applicant will have a strong knowledge and understanding of the issues surrounding the out-of-home care and services in Wimmera They will have strong communication skills along with the ability to provide support, guidance to the foster carers and clients the Out of Home Care program .To maintain a positive, enthusiastic and productive work environment.
