  • Full time, fixed term till 30 September 2019, with the potential to extend to 30 November 2019

About the role

The purpose of Psychosocial Transition Program (PTP) is to provide non-clinical community-based support for people with severe mental illness who have been participants in various Wellways' program (Partners in Recovery, Personal Helpers and Mentors Support, D2DL) while they transition to the National Disability Insurance Scheme (NDIS) or Eastern Melbourne PHN's funded Psychosocial Support Service (PSS), provided by Neami National.

 

In simple terms, psychosocial support provided through PTP focuses on supporting consumers to:

 

  • test eligibility to NDIS and other transition options
  • receive practical help (daily living/tasks)
  • be supported (mental health, physical health, education, work, housing, leisure, etc.)
  • get connected (with others, family, friends, peers, community, mainstream supports, other services).

 

Skills & experience

  • Relevant qualifications or commensurate experience in working in a support role for people living with mental health issues, generally a Certificate IV or above
  • Demonstrated experience and skills in working within programs for people with a serious mental health issues or their carers
  • Ability to work with diverse groups including Aboriginal and Torres Strait Islander, refugee groups and LGBTIQ groups will be highly regarded
  • Strong orientation and commitment to social inclusion and diversity
  • A current valid Driver's License and the ability to undertake travel for the role
  • Right to work in Australia
  • Satisfactory National Police Records Check, Working with Children Check and Disability Worker Exclusion Scheme (DWES) Checks prior to commencement are mandatory.
