Project Officer

In this role you will be responsible for providing support to mainstream general practice to improve access to private general practice for Indigenous people. This will include working directly with a defined number of mainstream general practices in the familiarisation of CTG initiatives; and supporting mainstream general practices in the provision of appropriate and timely care for Indigenous people in their region, especially those at risk of or who already have a chronic disease.

The position will be responsible for:

  • Lead the periodic monitoring and review of new strategic geographic target areas
  • Lead the Client Referral Pathways policy development and review processes
  • Conclude six monthly action plans with these partner practices, specifying inputs from each CTG Team member
  • Record on MMEx each client referred from each partner practice [NOT mainstream service provider] upon referral
  • Sign-up Practices to Partnership Agreements in the agreed priority locations

To be successful in this role, you will have:

  • Current C Class Drivers Licence (QLD)
  • Current Criminal History Check
  • Current Blue Card
  • Current First Aid and CPR Certificate
  • Vaccinations:
    • Hepatitis A & B
    • Diphtheria, tetanus and pertussis
    • Measles, mumps and rubella
    • Varicella (Chicken Pox)
