Our client is currently recruiting an Complex Disability Case Manager to provide support coordination to participants with complex needs and assist them in achieving goals as set out in their NDIS plans. This role focuses on addressing barriers and assisting participants to connect with supports and build capacity and resilience. The ideal candidate will have experience providing case management to compex clients with mental health challenges.
To be successful in this role your must have;
- Certificate in Disability and Community Services
- Knowledge of the NDIS framework
- Experience and skills as a Case Manager using targeted interventions to assist people to maximise independence
- Demonstrated experience with client assessment and risk management
- Strong communication skills both verbal and written
- High level attention to detail and follow-through
- Knowledge and experience with the NDIS portal and Carelink is desired
- Specialist knowledge in Dual Diagnosis, Mental Health, Youth or Family Support
- Current drivers license is essential
- Clear National Police Check and International if overseas for 12 months or more in the past 10 years
- Clear result on Victorian Disability Worker Exclusion Scheme
- Working with Children’s Check
Your responsibilities will include;
- Provide time limited specialist support coordination to address high level risk and assist the participant to connect with supports
- Ensure that customer outcomes are identified, documented and all required services are provided as per participant plan
- Develop intervention plans for implementation by disability and mental health support workers as required
- Review NDIS client plans and ensure all NDIS services are provided as per appropriate legislation, guidelines and funding network
- Ensure all relevant information is communicated clearly and concisely to all individuals involved in the participants plans
- Maintain participant information integrity in Carelink and ensure that all claimable services provided against a participant’s plan are rostered and approved by Carelink
