A little about the role

The Case Management Worker position in DPV Health Homelessness services works collaboratively with the Supported Referral program as part of the Northern Metropolitan Homelessness Network.  The position utilises practice skills within program guidelines and practice standards.  It adheres to continuous quality improvement processes for people in the program catchment who experience homelessness or are at risk of becoming homeless.

About you

Applicants must have:

  • Relevant Australian tertiary qualification in social work, community services, welfare studies or other equivalent and related discipline;
  • Demonstrated knowledge of the Specialist Homelessness Services (SHS), Supported Accommodation Assistance Program, the homelessness service system and current developments including the DHHS Standards, Continuous Quality improvement and the Opening Doors Framework;
  • Demonstrated case management experience within the homelessness sector or other relevant community settings;
