About Us?
The Institute for Urban Indigenous Health (IUIH) leads the development, planning and deliver of comprehensive primary health care for Aboriginal and Torres Strait Islander people living in South East Queensland.

The IUIH Connect Manager position’s purpose is to reduce the significant points of risk in a person’s journey through the health care system, in particular hospital entry, discharge and transit. The Scope of this position includes business planning, leadership, program management and quality control.  Your primary accountabilities will include management and development of a coordination service, focused on addressing the gaps in health care for our Aboriginal and Torres Strait Islander Community.

About the Role:

  • You will be required to travel to North and South Brisbane 
  • Work in partnership, liaise with and consult with key stakeholders; Government departments, hospitals, PHN, for maximum coordination between stakeholders
  • Seek additional funding opportunities in line with IUIH Action Plans
  • Develop networks in local Indigenous Communities to build community awareness and engagement with the IUIH Connect hub and program
  • Coordinate support for discharge planning including transition care, coordination and follow up in the community

Essential:

  • Demonstrated management experience in a comparable not for profit organisation
  • High level communication, Networking and advocacy skills
  • Current C Class Drivers Licence
  • Current Blue Card
  • Current First Aid / CPR Certificate
  • Relevant qualification in Nursing, Community Services or Primary Health Care

Desirable, but not mandatory:

  • Previous experience working with Aboriginal and Torres Strait Islander community
