Dietitian – Grade 2

Part Time, Fixed-term position available for a Grade 2 Community Health Dietitian

A little about the role

This is a newly established, fixed-term position, suitable for an experienced Dietitian. You will be working alongside allied health medical and staff to deliver best practice care to people within the DPV catchment areas of Hue and Whittlesea. This role is based within the Hume region, though travel across sites will be required.  

Join our dynamic, multi-disciplinary team to provide assessment, education and self-management support services to our diverse community. 

Your day to day responsibilities will include:

  • Providing evidence based, client centered, goal directed assessment and care planning to a diverse caseload of clients
  • Contributing to the multi-disciplinary team, which may include team-based care plans, case reviews and initiating referrals as appropriate
  • Meeting organisational and funding requirements and KPIs
  • Communicating effectively with clients, colleagues and other stakeholders

About you

You will have experience working within a community setting. You will also have:

  • Qualifications in Nutrition and Dietetics, and be eligible for the Accredited Practising Dietitian Program. Experience in community health settings and working with paediatric clients will be highly regarded  
  • Well-developed clinical skills and demonstrated ability to support a coordinated approach by working with  a range of stakeholders to improve  health outcomes as agreed with client,
  • Experience in planning, implementing and evaluating innovative, evidence based, clinical services
