Transitional Support Services Team Leader

About Us?

The Institute for Urban Indigenous Health (IUIH) leads the development, planning and deliver of comprehensive primary health care for Aboriginal and Torres Strait Islander people living in South East Queensland.



We are currently looking for an experienced individual who is interested in providing leadership to our Transitional Support Services team. The team consists of two programs providing transitional care coordination to clients within the inner city region and prison transition care coordination in the last 6 weeks of custody and supporting integration and transition back into the community.

About the Role:

  • Provide effective coordination and leadership to a team of transitional support care coordinators and case managers
  • Engagement and collaboration with key stakeholders and services to deliver client centered support and care planning across medical and social support needs
  • Assist with the planning and implementation of support programs tailored to the individuals, families or carer’s using the intervention model care approach
  •  Leadership in ongoing development of program enhancements and service efficiencies.

Essentials:

  • Minimum of 4 years within the social services sector
  • Bachelor of Health Services, Social Services or relevant qualification
  • Criminal History Check (willing to obtain)
  • Driver’s License and the ability to travel across SEQ as required
  • Demonstrated leadership with a focus on growing our own
  • Ability to work sensitively and effectively with Aboriginal and Torres Strait Islander people in a fast paced environment
