About the role

 

The Young Carer Assessment and Planning Officer (YCAPO)  is a new position created to service the needs of Young Carers aged between 8 and 25 years old.  

This is a full-time permanent position and works in collaboration with other Carers WA Youth Team members.  As young carers are identified internally / externally the YCAPO is able to provide a comprehensive individualised assessment using the Carers Star and with the young carer, design a Carer Directed Support Plan to support them in their role as a young carer. Carers Star training will be provided.

As a social worker with previous youth experience including under 18's, you will be able to use your knowledge and experience to engage, assess and guide the young person and their guardian  / care recipient  through the registration and assessment process.  Assessments may be conducted by outreach (school / home environment) or via phone / zoom depending on the needs of the young carer.

This role is aimed at early intervention planning i.e. NOT crisis family services.

Applicants MUST be able to register with Australian Association of Social Workers (AASW) and have a minimum of 2 years' experience working with under 18's.

Skills and experience

 

Experience in providing advisory, assessment and/or planning processes for youth case management referral processes. 

Understanding and empathy of carers, health, mental health, ageing and disability issues and their correlation with social and educational disadvantages of youth (Carers Recognition Act 2004)

Understanding of youth participation principles and a commitment to implement these in accordance with Carer WA's vision, mission and values

Current (or able to obtain) Mental Health First Aid and / or Applied Suicide Intervention Skills Training (ASIST) certification

Significant experience using computer software including Microsoft Suite, CRM and/or customised client databases.

Flexible, creative and compassionate style delivery to reduce anxiety whilst balancing documentation and contract requirements.

Well-developed communication skills (verbal and written) with problem solving, advocacy and negotiation skills.
