About the Role:

We have a rare and fantastic part time (3 days per week) opportunity for an experienced Social Worker to provide evidence based and consultative social work support across our service centres, managing communication and connectivity between internal and external stakeholders.

Montrose has a strong history of providing social work services to our community. We are heavily committed to supporting our therapists and families in a range of different areas.

In this role you will join our multidisciplinary team of Social Workers, Speech Pathologists, Occupational Therapists and Physiotherapists who provide evidence informed and contemporary therapies for our clients.
Our multidisciplinary and interprofessional approach means our therapists work together so clients can get the most out of their therapy and reach their goals successfully.

The role of our Social Worker is to provide mentoring and support to Montrose therapy teams, our clients and their families across all service centres, particularly in relation to complex case management through provision of an inclusive and customer centric experience. This role also works closely with our Short Stay and Lifestyle & Leisure Coordinators to assist with goal directed group program planning.

You will also have a demonstrated referral partner network to draw upon and the capacity to build/strengthen relationships with other relevant organisations to facilitate both interim and ongoing support for our families where required.

There is travel involved across our service centres. We provide company vehicles for travel but you may also be required to use your own vehicle at times.

Our centres are based in Brisbane South (Darra), Brisbane North (Kedron), Sunshine Coast (Mooloolaba), Gold Coast (Arundel), Slacks Creek, Ipswich and Toowoomba. This role is based at Darra but will require travel to other centres or client homes as required.

Key Responsibilities:

  • Meeting discipline specific standards of practice
  • Achievement of customer goals as per Goal Achievement Plan (where appropriate)
  • Mentoring and support of colleagues including outcomes and recommendations
  • Participation in case conferencing
  • Support organisational culture through our values and behaviours
  • Active participation in ongoing learning activities and professional development
  • Effective and collaborative communication with customers, team members and others.

Essential for this role are the following:

  • Previous experience in disability services, or other relevant community services sector;
  • Social Work and/or Human Services degree or other relevant qualifications;
  • Membership with Social Work Association;
  • The ability to provide staff training and support on job related training needs;
  • Excellent customer relationship management and a proven ability to manage customer service across multiple stakeholders within an organisation;
  • Work collaboratively with colleagues in a multi-disciplinary services environment;
  • Experience in mentoring of staff;
  • Demonstrated clinical competencies;
  • Excellent administrative and communication skills;
  • Ability to travel to all service centres across Montrose to provide support for staff members and families;
  • Availability for on call support on occasion;
  • Current ‘C’ class driver’s license.
  • Computer literacy in Microsoft Office Suite.
  • Current working with children (blue card) and be eligible to apply for a working an exemption notice (yellow card) through The Department of Communities, Disability Services and Seniors.
  • Provide a certificate of completion for the NDIS Worker Orientation Module.
  • Provide a certificate of completion – Australian Government Infection Control – COVID-19.
  • Be an Australian or NZ Citizen or Permanent Resident, or have a relevant visa
