Case Manager

The Opportunity
The Case Manager will provide intensive case management support to people experiencing homelessness or at risk homelessness. You will drive responsive and client-focused services by encouraging active client participation and individual learning outcomes by engaging the client with the larger community and other relevant agencies.


Key responsibilities include:

  • Manage referrals, assessing needs and providing appropriate services
  • Deliver engaging and relevant activities and programs that deliver relevant service outcomes
  • Deliver a service that acknowledges diversity and the impact of multiple and complex needs of people experiencing homelessness or at risk of homelessness
  • Advocate on behalf of, and maintain the rights of people experiencing homelessness or at risk of homelessness
  • Be an active member of the staff team and positively contribute to a supportive team environment

We are seeking a dynamic individual who holds a tertiary qualification in Human Services or equivalent and experience in working with marginalised people, including skills in case planning and complex case management.

The successful candidate will possess sound interpersonal skills, and extensive knowledge and commitment to principles of social inclusion and quality management.
