Case Manager/Client Advisor- Aged Care

  • Locum role,commencing ASAP
  • Leading Not for Profit Aged Care Provider - Home Care Packages
  • Full time hours- Work from home

As Client Advisor for this highly regarded Not for Profit In Home Care provider, your key role will be to develop and nurture positive interactions between clients, carers and local service providers.

You will be working from home during COVID-19 but will be required to complete client visits for certain clients.

To be successful in this role you must have:

  • Experience developing Care Plans for complex clients
  • Current Victorian Drivers Licence
  • Current National Police check clearance
  • Tertiary qualifications in Community Services or related disciplines
