Plan Partners Support Coordinator

As a Support Coordinator, you'll work with our customers, their families and carers to coordinate all aspects of their NDIS journey. You will help customers to understand their NDIS plan, set up service agreements with providers, prepare plan reviews, conduct risk assessments and, critically, provide support should issues arise.

Using your high level communication skills and industry experience you will build and leverage networks to ensure the best support for your customers. You will rely on your time management ability to allocate your billable time appropriately to achieve your targets and at the same time ensure strict compliance to all reporting and auditing requirements.

To succeed as a Support Coordinator with Plan Partners you will have:

  • Past experience in case management or practical experience within the disability sector
  • Working knowledge of the NDIS
  • A drive to contribute to the success of Plan Partners and go the extra mile for our customers
  • A high level of comfort working with targets, and ideally experience with a billable hours model
  • High level interpersonal skills, time management skills and the ability to multitask
  • A valid driver's licence, and your own reliable vehicle
