About the role

 

As an NDIS Support Coordinator, you will work closely with National Disability Insurance Scheme (NDIS) Participants to understand their NDIS plan, engage with appropriate service providers and allied health professionals, and monitor progress toward the achievement of their plan goals.

This position is primarily for, but not limited to, providing Support Coordination to NDIS Participants who are Deaf or Hard of Hearing.

Role requirements include, but are not limited to, the following:

  • Developing strong relationships with NDIS Participants and their families/carers to assist them to meet their goals and get the best out of their NDIS plan;
  • Facilitating the implementation of all supports identified in a Participant's plan;
  • Building and developing strong relationships with a range of service providers and allied healthcare professionals;
  • Providing pre and post NDIS Planning support;
  • Strengthening the Participant's ability to navigate the NDIS and coordinate their supports;
  • Plan review reporting and meeting support.

About you

 

To be successful in this role you will need to demonstrate your ability to meet the Selection Criteria outlined below:

  • Fluency in Australian Sign Language - AUSLAN;
  • Relevant tertiary qualification or equivalent and/or significant experience relevant to the role;
  • Sound understanding of the National Disability Insurance Scheme;
  • Demonstrate customer service skills with commitment to ensuring a positive, professional customer experience;
  • Understanding of the needs of people with a disability and their families/carers;
  • Highly developed written communication and interpersonal skills, negotiation skills, and ability to develop strong relationships with all stakeholders;
  • Strong time management and organisational skills with ability to plan work effectively while demonstrating flexibility to meet individual needs;
  • Demonstrate problem-solving skills to successfully identify issues and barriers, develop and implement solutions in collaboration with Participants;
  • Proficient computer skill level, including the use of Microsoft Office programs;
  • Proven experience in record keeping including an ability to maintain confidential information and maintain program paperwork;
  • Proven ability working to targets and KPI's; 
  • Demonstrated ability to be able to work independently and as part of a team;
  • Current Victorian drivers licence & ability to utilise own vehicle.

  • Working with Children Check
  • Police Check (International Police Check if applicable) 
  • Disability Workers Exclusion Scheme Check
