About the Role:
A Harp Care Coordinator is a Registered Nurse Grade 3B who is appointed to provide case management services to clients and their carer’s who have a chronic disease or complex needs and are frequently admitted to the hospital or present to the Emergency Department.
You will be responsible for coordinating programs in the hospital and community setting that is directed at reducing avoidable hospital admissions and presentations to the Emergency Department.
As a Harp Care Coordinator you must be a Registered Nurse who is registered with AHPRA. You must demonstrate that you have consolidated your knowledge, clinical skills and have a minimum of 4 years post graduate experience. It is essential that you have to have the ability to assess a patient’s clinical, psychological and social wellbeing and to have the knowledge to implement the required care coordination required. You will need a sound understanding of the hospitals internal and external support networks.
You will have the experience and skills to manage clients with chronic disease. You will demonstrate that you have the ability to make decisions independently and able to work collaboratively in a team environment. You will have well developed communication and interpersonal skills & the ability to liaise with the client, families & other health professionals. You will possess strong time management skills and the ability to adhere to strict time lines.
A current Drivers Licence and police check are essential for this role.
A post graduate tertiary qualification is desirable in Coronary Care or equivalent, or Chronic Disease Management. You must be prepared to maintain & participate in ongoing education & professional development to maintain knowledge of chronic disease management.
