BACKGROUND

Since 1987, ACES has been providing programs and services to Aboriginal Elders. ACES was incorporated under the Associations Incorporation Act (1981) and have a Board of Management made up of elected community members.

In the Australian context, ACES is unique in being both Aboriginal Community Controlled and specialising in provision of both a planned activity group and residential aged care services.  At ACES culture is recognised as central to individual and community health. Further the integration of a range of programs on the site ensures continuity in the assessment and delivery of appropriate care to Aboriginal Elders in a culturally appropriate environment as they continue through their life journey.

ENVIRONMENT

Amidst the hustle and bustle of East Brunswick, ACES nestles in the serenity of lush, well maintained, bush-like gardens and the Merri Creek. The setting provides Elders with a friendly, relaxed culturally relevant home-like environment which is important to their health and well-being.

1.         POSITION DETAILS
 

The main purpose of this role is the planning, coordination and delivery of ACES Koori community care (KCACPs) program.

 

The position will also play a key role in establishing effective partnerships and networks which will contribute to Indigenous aged care clients receiving an appropriate level of community care.

 

2.        POSTION OBJECTIVES

 

  • Ensure the KCACP operates in accordance with the requirements of the ACES Funding Agreement, relevant legislation including the Aged Care Act 1997, the Information Privacy Act 2000 and the Health Records Act 2001, and the National Aboriginal and Torres Strait Islander Flexible Aged Care Program Service Provider Guidelines developed by the Australian Department of Health and Ageing and also the eight (8) Aged Care Quality Standards as set by the Aged Care Safety and Quality commission.
     
  • Receive, screen and prioritise referrals (intake) for the KCACP in accordance with ACES KCACP’s policy and procedures. Maintain the Program waiting list when appropriate.
     
  • Ensure that each client has a current ACCR (Aged Care Client Record), service agreement, and care plan which defines their needs and the nature, level and extent of services required.
     
  • Ensure services are provided to care recipients in a manner that respects their privacy, dignity and culture; and the confidentiality of care recipients’ records and personal information is maintained.
     
  • Promote the ACES KCACP service to Aboriginal community members.
  • Work closely with ACAS and the range of other stakeholders involved in the care of KCACP clients and their families.
  • Work within program systems and complete tasks in accordance with work instructions, annual Work plan, direction by the ACES KCACP Manager and the ACES KCACP Manual.
  • Participate in continuous improvement strategies and activities.
  • Promote a safe workplace environment and facilitate initiatives to enhance safety.
  • Undertake data entry in particular, input client information, case notes and work performance information into the Care Manager software.
  • Undertake client case management including:
  • Developing Care Plans, undertaking reviews and organising reassessments for clients in accordance with NATSIFACP Guidelines.
  • Developing Care Plans in direct consultation with clients and carer/family, outlining care objectives, services to be provide, level of service, tasks etc.
  • Reviewing client Care Plans every twelve weeks or more frequently as needed.
  • Ensuring that the range and levels of service being delivered to clients are within the approved program guidelines and budget constraints.
  • Participating in inter-agency care plan meetings.
  • Monitoring service provision and service costs for individual clients.
  • Communicating outcomes of assessment and reassessments to other service providers when relevant and with the Elders consent.
  • Maintaining detailed case notes on each client, which are accurate, up to date and reflect all contacts and note all changes observed or reported.
  • Undertaking comprehensive assessments, re-assessments and reviews of KCACP clients within 5 working days of a referral /request for a review.
  • Ongoing monitoring, recording and follow up of clients’ well-being, their care needs, supports and services.
  • Providing counselling information and advocacy to clients and carers as required.
  • Making assisted referrals when required.
  • Liaising with relevant service providers to ensure optimal Elder consistency is achieved.
  • Liaising with all relevant groups/agencies to ensure efficient and effective care services.
  • Providing responsive and flexible care management to Elders of the Koori Community Aged Care Program including liaison with relevant carers, providers and agencies.
  • Initiating case planning meetings as required.
  • Undertake other duties as specified by the KCACP Manager.

 

                                                                                           

3.       KEY DUITES AND RESPONSIBILITIES

 

Selection Criteria

 

  • Knowledge and understanding of or the ability to rapidly acquire knowledge and understanding of the Koori Community Aged Care Program.
  • Case Management experience and the ability to provide responsive and flexible care management to clients of the Koori Community Aged Care Program including liaison with relevant carers, providers and agencies.
  • Proven ability to work well both within a team and independently.
  • Effective written and oral communication skills.
  • Demonstrated organisational and time management skills.
  • Knowledge and appreciation of the Aboriginal and Torres Strait Islander culture and an ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people.

 

Mandatory

 

  • Certificate 4 in Community Services or Certificate III in Individual Support (Ageing and Home and Community Streams)
  • Current First Aid Certificate
  • Computer skills, word, power point and excel
  • High Level oral, written skill and communication skills 
  • Current National Police Certificate
  • Current Victorian License.

 

Desirable 

  • Experience in similar role
