Telephone Health Coach/Diabetes Educator

1.0 FTE (Hobart, Launceston or Devonport based)

Diabetes Tasmania is a not-for-profit organisation whose purpose is to work with our community to prevent and reduce the impact of diabetes. We work collaboratively across all sectors to raise awareness and advocate for all people affected by diabetes. 

We are seeking applications for a combined telephone health coach and diabetes educator position.  

This is a unique opportunity to work across the full spectrum of diabetes care offered by the clinical services team. Most of the role will be delivering telephone health coaching using The COACH Program®.  A smaller component will be delivering individual diabetes education in the community. 

In addition, there will be opportunities to participate in group education and awareness programs and to support the youth programs, such as camps or day activities for young people with diabetes. 

The successful applicant will be working as part of a multidisciplinary state-wide team which includes diabetes educators, dietitians, a social worker and diabetes nurse practitioner.
